Health Forms

Health forms are necessary for all scouts and all parents and siblings that attend any camp out. They need to be filled out once every 365 days, which is why sometimes depending on camp-out dates each year it appears we need this filled out TWICE each year.

Step 1: Print this form for each member of your family => http://www.scouting.org/filestore/HealthSafety/pdf/680-001_AB.pdf

Step 2: Fill out the form and leave out the date and your signature

Our Pack is part of the Capitol Area Council

Suggestion: Make multiple copies without signing and dating, then later its easy to add any updates and sign

Step 3: Photo copy your health insurance card for each application

Step 4: Sign and date a copy of each form and give them to your den leader
